Job Requirements

• 3 years of recent ED acute, inpatient, or hospital case management experience within the last 5 years required
• Level I Trauma experience as a nurse or case manager required
• Experience with inpatient medical, surgical, emergency department, and pediatric populations required
• Active RN license required; pending license may be considered if application has been submitted to the licensing board
• BLS certification required
• ACLS certification required
• Previous travel nursing experience preferred
• Epic EMR experience highly preferred

Responsibilities

• Provide RN case management support for adult inpatient and emergency department patients
• Coordinate patient care plans, discharge planning, and transitions of care across the continuum
• Collaborate with interdisciplinary teams to support quality outcomes and efficient resource utilization
• Assess patient needs and help facilitate appropriate services, referrals, and follow-up care
• Maintain accurate and timely clinical documentation within the electronic medical record
• Adapt to unit needs and float within scope when necessary
• Support patients and families through effective communication, education, and advocacy
