Join a dedicated home health care team as a Physical Therapist (PT) serving patients throughout Lakewood, Denver, and surrounding Colorado communities. In this rewarding home health physical therapy contract, you will help patients improve mobility, regain independence, and safely manage daily activities in the comfort of their homes. The work area is based in Lakewood, Colorado (ZIP code 80228) and includes patient visits throughout Lakewood, Denver, and surrounding Colorado communities, providing a meaningful opportunity to deliver care in patients’ homes. The position offers weekday hours and no on-call requirement.

Home Health Physical Therapist Job Details

• Location: Lakewood, Colorado, with travel throughout Douglas, Jefferson, Arapahoe, Adams, Boulder, Weld, Broomfield, and the Denver metropolitan area
• Weekly Estimated Pay: $1,880-$2,020
• Contract Duration: 13 weeks, from October 26, 2026 through January 24, 2027
• Schedule: Monday through Friday, 8:00 a.m. to 5:00 p.m.; no on-call requirement
• Shift Duration: 8-hour shifts
• Hours: 40 hours per week
• Openings: 2
• Productivity Expectation: 30 points per week

Physical Therapist Job Requirements

• At least 1 year of professional Physical Therapist experience; home health experience is required
• Current Colorado Physical Therapist license; compact licensure accepted
• Experience with home health documentation and electronic clinical systems, including HCHB
• OASIS assessment experience
• Strong clinical judgment, communication, organization, and time-management skills
• Ability to travel independently between patient visits across the assigned service area
• Commitment to patient-centered care, safety, and accurate clinical documentation

Home Health Physical Therapist Responsibilities

• Provide direct and indirect physical therapy services to patients in their homes and other approved community settings
• Evaluate patients’ functional abilities, mobility limitations, safety concerns, and rehabilitation needs
• Develop, implement, and update individualized treatment plans in accordance with physician-approved plans of care
• Administer therapeutic exercises and other skilled interventions to improve strength, balance, coordination, mobility, and activities of daily living
• Instruct and motivate patients and caregivers on home exercise programs, safety techniques, and strategies for maximizing independence
• Complete OASIS assessments and maintain timely, accurate documentation in HCHB
• Recommend and support the appropriate use of adaptive equipment and durable medical equipment
• Provide clinical supervision and oversight to Physical Therapist Assistants and Home Health Aides as appropriate
• Collaborate effectively with physicians and the interdisciplinary care team to promote coordinated patient outcomes
