Job Details

• Location: Assigned area in Kentucky
• Setting: Home care, in-home wellness visits, and risk adjustment assessments
• Start Date: 9/7/2026
• Assignment Length: 52 weeks
• Schedule: 4 shifts per week
• Shift Duration: 8-hour days
• Hours Per Week: 32
• Travel: Field-based role with regional travel throughout the assigned area and state

Job Requirements

• 3 years of patient care experience preferred
• Must have a valid, unencumbered Nurse Practitioner license for the state you will be working in
• Experience in primary care, adult care, geriatric care, homecare, or related clinical settings preferred
• Previous in-home risk assessment or home health experience preferred
• Comfort working independently in the field
• Strong documentation and EMR skills
• Reliable transportation for travel to patient homes within the assigned territory
• Excellent communication, assessment, and patient education skills
• Bilingual skills are a plus

Responsibilities

• Perform annual in-home wellness visits and health assessments for Medicare members and other at-risk populations
• Conduct comprehensive evaluations, including physical assessment, vital signs, medication review, functional status review, pain assessment, cognitive screening, and fall risk screening
• Assess the home environment and social factors that may impact patient well-being
• Document findings thoroughly and accurately to support risk adjustment and quality care initiatives
• Provide patient education related to medications, chronic conditions, preventive care, and overall wellness
• Help identify and close care gaps, including recommended screenings and labs
• Build trusting relationships with patients through empathy, professionalism, and clear communication
• Collaborate with the broader care team to enhance care delivery and patient outcomes
• Travel throughout the assigned region, typically within a 55-mile radius, with additional travel coordinated in advance as needed
