Home Health Physical Therapist Job Details

• Weekly Estimated Pay: $1,990-$2,140
• Location: Florence, South Carolina, with home health visits across Lancaster and Chester counties
• Employment Type: 13-week travel contract
• Start Date: October 12, 2026
• End Date: January 8, 2027
• Schedule: Monday-Friday, 8:00 a.m.-5:00 p.m.
• Shift Duration: 8 hours
• Guaranteed Hours: 40 hours per week
• On-Call: Not required

Physical Therapist Job Requirements

• Current Physical Therapist license eligible for practice in South Carolina
• At least 1 year of professional Physical Therapy experience
• Previous home health Physical Therapist experience required
• Experience with HCHB documentation preferred or required for workflow readiness
• Strong clinical judgment, communication, organization, and time-management skills
• Ability to travel between patient homes and provide skilled physical therapy in alternative care settings

Home Health Physical Therapist Responsibilities

• Evaluate patients in their homes and identify functional, mobility, and safety needs
• Develop and implement individualized physical therapy treatment plans based on physician-approved plans of care
• Provide therapeutic exercises and skilled interventions to improve strength, balance, mobility, and activities of daily living
• Instruct, motivate, and support patients and caregivers in achieving functional goals
• Recommend and support the appropriate use of adaptive equipment
• Maintain accurate clinical documentation and meet a productivity expectation of 30 units per week
• Provide clinical supervision and oversight to Physical Therapist Assistants and Home Health Aides as assigned
• Communicate effectively with physicians and the interdisciplinary care team to promote coordinated patient care
