Home Health Registered Nurse Requirements

• Current Registered Nurse (RN) license valid for practice in Texas
• Associate degree in Nursing or higher
• At least 1 year of professional nursing experience; home health experience is preferred
• Ability to provide safe, independent skilled nursing care in patients’ homes
• Strong patient assessment, clinical judgment, documentation, communication, and organization skills
• Reliable transportation and ability to travel throughout the assigned New Braunfels service area
• Homecare Homebase (HCHB) experience is a plus

Travel Registered Nurse Responsibilities

• Provide skilled home health nursing care according to individualized plans of care.
• Perform comprehensive patient assessments, monitor conditions, and identify changes requiring intervention.
• Administer medications and treatments, complete nursing procedures, and provide disease-management education as ordered.
• Coordinate patient care with physicians, therapists, caregivers, and other members of the interdisciplinary healthcare team.
• Document home health visits, assessments, interventions, and patient progress accurately and promptly.
• Educate patients and families on medications, symptom management, safety, and strategies that support independence.
• Participate in rotating on-call and weekend coverage as scheduled.
• Travel between patient visits while maintaining productivity and quality-of-care expectations.
