Physical Therapist Job Requirements

• Degree in Physical Therapy from an accredited program
• At least 1 year of outpatient physical therapy clinical experience
• Active Physical Therapist license valid in Indiana
• Current CPR certification

• Commitment to patient safety, professional standards, and applicable Indiana practice requirements

Outpatient Physical Therapist Responsibilities

• Complete comprehensive physical therapy evaluations of assigned patients and identify functional limitations, goals, and treatment priorities.
• Develop, implement, and modify individualized treatment plans based on patient needs, physician direction, and progress toward discharge.
• Provide discipline-specific outpatient physical therapy services designed to support a safe and effective return to community activities.
• Document evaluations, treatment plans, progress, goals, functional outcomes, medical necessity, and charges accurately and promptly.
• Deliver patient and family education regarding treatment programs, home exercises, safety, and continued functional improvement.
• Collaborate with the healthcare team and provide appropriate direction or delegation of therapy-related services.
• Maintain a safe treatment environment by following clinic procedures for patient handling, infection prevention, equipment use, and hazard reporting.
• Participate in quality-improvement initiatives and pursue ongoing professional development to strengthen clinical practice.
