Physical Therapist Job Requirements
- Active Physical Therapist license in the state of Ohio
- Bachelor’s degree required
- At least 1 year of home health physical therapy experience
- Experience with HCHB is a plus
- Strong communication, organization, and patient education skills
- Ability to work independently while delivering high-quality care in a home-based setting
Responsibilities
- Provide skilled physical therapy services to home health patients
- Develop and implement individualized plans of care
- Deliver gait and balance training to support patient mobility and safety
- Guide strengthening exercises tailored to each patient’s goals and condition
- Perform home safety assessments and recommend modifications as appropriate
- Document patient progress accurately and timely
- Collaborate with the care team to promote positive patient outcomes
- Support patients in achieving greater independence and improved quality of life
